Application Procedure

• A minimum deposit must accompany each application. (See specific camps for full cost.)

• Make checks payable to Michigan Tech. A convenience fee of $6 will be assessed on all phone, fax, mail and internet payments. Please allow two to three weeks for processing.

• A $20 fee will be charged for all returned checks.

• All participants must submit an application and health history form prior to attending camp. Participants without a completed health history form (signed by a parent or legal guardian) will not be able to participate in the program. It is also recommended that each participant provide the Summer Athletic Programs with a copy of a current physical.

• Basketball Team Camp: Coaches are encouraged to submit all team applications no later than one week prior to start of camp. Summer Athletic Programs cannot guarantee teams placement in the camp or accommodations if information is not received in a timely manner.
  

• Outstanding balances are due June 4, 2010. Payments made after June 4, 2010, will be assessed a $25 late fee.

• Registration is not guaranteed until all forms are received and balance is paid in full.

• No discount is given for late arrival or early departure.

Cancellations

The following conditions apply to all refunds:

• Cancellations prior to June 4, 2010 - Refunds, minus a $50 cancellation fee, will be issued upon receipt of a letter of explanation from a parent or guardian.
• Cancellations after June 4, 2010 - Refunds, minus a $50 cancellation fee, will be issued for medical reasons only. A doctor's certificate must accompany the letter of explanation from a parent or guardian.
• Injuries - If a participant is injured during camp, a refund will be issued in the form of a credit voucher for 2011 for 80 percent of the unused portion of camp.
• Discipline - No refunds are given when a participant is expelled for disciplinary reasons.

Refund checks may not be issued until the conclusion of the Summer Athletic Programs in late August.
